In order to reduce legal and ethical risk to the therapist when a client requests a document or report to be released to them, such as their SDI report, other assessments and instruments, or file summaries, it is important that the therapist have a signed release of liability document for clients who are requesting any information or materials to be released by the therapist into the client's own care.
The Clinical Documentation Liability Release Form is a two page document that Mari A. Lee, LMFT, CSAT-S is making available at no charge to licensed therapists and/or Certified Sex Addiction Therapists as a support to fellow licensed clinicians. This is formatted in a downloadable word document so that the clinician can easily edit with their own practice information and tailor to their practice needs.
