FREE Speakers Survey Feedback Form

If you are a speaker and are in need of an attendee survey and feedback form to support and enhance your presentations, Mari has provided this free form from her own files as a public speaker as a support to therapists, small business owners, and other colleagues.

Please note that this form does not include the clinical therapy forms, consents, sign off pages, or materials packet created to support legal and ethical documentation and policies of private practice clinical professionals. You can find that material HERE. 

In order to grow as a speaker and presenter is wise to request feedback from the attendees. Additionally, asking for signed permission to use positive comments on your speakers page, marketing materials, or for website testimonials is an important and ethical step.

The Speakers Survey and Feedback Form is a document created by Mari A. Lee, LMFT, CSAT-S for her own public speaking presentations. Mari is making this available at no charge to licensed therapists and public speakers. This form is formatted in a downloadable Word document so that the professional can easily edit with their own practice or business information and logo and tailor to their practice needs.
